What you need to know
Life Focus, Inc. is pleased to accept Aetna, BCBS/Anthem and UHC. If you would like to utilize your in network benefits, please contact your insurance provider to verify that I am listed on your plan. I can be found in the provider directory with Aetna, BCBS/Anthem, or UHC under Edward Clark, Jr. I do not accept Medicare/Medicaid. Please contact your insurance company and ask the following questions:
- Do I have mental health insurance benefits?
- Is preauthorization required?
- Do I have a copay or deductible? Has the deductible been met?
- Do I have a maximum number of visits per year?
It is our goal to make every attempt to verify benefits prior to the first visit. We ask that you download and completely fill out the LFI-Insurance Financial Policy form. Please have EACH person being seen complete this form (not one form per couple/family). Please email this form, a copy of the front and back of your insurance card, as well as a copy of your driver’s license (to prevent fraud) to firstname.lastname@example.org. Please allow us approximately 24 hours to verify your benefits. Verification of benefits does not guarantee payment. The actual payment of the claim by your insurance company will determine member responsibility, which may be different than what was originally estimated due to various things such as procedure and/or diagnosis codes, secondary insurance (if applicable), your eligibility and the details of your plan. *Please note that most insurance companies do not cover marriage therapy and you will be required to pay the full fee if the claim is denied. If benefits can not be verified prior to your appointment, you will be expected to pay the full rate for the first visit. If there is an overpayment, you will receive a credit towards your next appointment. Ultimately, you are responsible for any services not covered by your insurance.
It is your responsibility to submit a new form if your insurance changes in the middle of the year. I also ask that you submit a new one at the beginning of the year.
If you have other insurance, Life Focus, Inc. is able to provide you with a service summary receipt in order for you to submit to your insurance provider for possible reimbursement. Life Focus, Inc. does not submit ANY claims to any health insurance provider unless we are considered an in network provider.
Cash, check and all major credit cards accepted for payment. However, only cash and checks are accepted for Collaborative Divorce clients.
If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session (not the insurance co-pay). Insurance does not pay for missed appointments therefore a claim will not be submitted.
